My Physicians

While it might be obvious who your physician (healthcare provider) is, consider including all your health-related providers in this section of CladeHealth Tracker. Imagine having all their contact information in one, easy to find location.

First, of course, should be your primary care provider. This might be a physician, naturopath, or perhaps a family/nurse practitioner. If you belong to a clinic or medical practice group where the practitioner might change from visit to visit, list the practitioner you see most often but include the general office number (and not the individual practitioner’s phone number). If you sometimes see a physician, and on other visits see a nurse practitioner, add both names and contact information.

Do you see a specialist for any of your healthcare, such as a cardiologist? What about a midwife or physical therapist? Include these specialists as well! What if you were to break a tooth while away from home? You will know exactly where to find the contact information for your dentist.

Here is a list of specialists to consider adding (hint: think of specialists you’ve seen more than once, or might continue to see again—someone who provides an important part of your ongoing healthcare):

Don’t forget you can add up to 12 individuals on CladeHealth Tracker (you, your partner, children, parents, etc.). If you have added your children to your CladeHealth Tracker user list, consider adding their pediatrician, nurse practitioner, and/or any specialists, and their school nurse too.
